The protagonist wakes up inside a giant dimension-crossing illithid flying ship, infected with a parasitic tadpole that transforms people into mind flayers. The transformation doesn’t happen right away. After a githyanki attack on the nautiloid, it crashes in Faerûn, leaving the player stranded with new telepathic abilities. The overarching villain is an illithid elder brain known as the Absolute. Three antagonists control it Ketheric Thorm, voiced by J.K. Simmons, Enver Gortash voiced by Jason Isaacs, and Orin the Red voiced by Maggie Robertson. The party scours the Gauntlet of Shar, a deserted temple to the goddess of darkness, and later confronts Gortash and Orin in the city of Baldur’s Gate itself. As you get closer to the city, residents comment on your actions. The local Gazette updates with your feats. The world actually notices you.

With the looming threat of war and a mind flayer invasion on the horizon, friendships are forged on your journey. Each companion has their own moral compass and reacts to the choices you make. Key companions include Lae’zel, Shadowheart, Astarion, and Gale, each with deep backstories that affect the story. Some will approve of your darkest choices. Others won’t say a word but you’ll feel it.
